FTD Point of Purchase Kit: Opt-Out Form

The Point of Purchase (POP) kit includes posters of FTD holiday codified products and a workroom calendar (sent in the Fall/Winter kit only). Periodically other marketing materials may be included in the kit.

After you have reviewed the terms and conditions, please complete the "opt out" request form.

If you have already opted out of receiving any POP Kits in the past, you will remain on the opt out list for any future mailings. If you would like to opt back in, please contact Member Services at 800.788.9000.

Terms and Conditions
Here is how the POP Kit fulfillment works:

  1. If you purchase any FTD holiday codified containers you will receive a POP Kit FREE of charge.
  2. If you DO NOT purchase any FTD holiday codified containers you will be charged $19.99 for the POP Kit on your FTD Clearinghouse Statement.
  3. If you do not wish to receive the POP Kit and do not want to be charged $19.99, simply opt out by completing the form below.
  4. If you choose to opt out of the POP Kit, you will not be shipped the POP Kit again until you contact FTD.
